Online Pre-Admission Form

In order to make your admission to our hospitals as speedy and hassle free as possible, you can now complete your pre-admission form online. Alternatively you can complete this pre-admission form as comprehensively as possible and then fax your completed form to us at the numbers listed below. You may, of course, also hand these in at the relevant pre-admission clinic at our hospitals, a list of which also appear on this site.
On your admission to hospital, you will simply be required to confirm your information and agree to the Conditions of Admission (these are available on this site) by signing the admission form.
We recommend that you attend the pre-admission clinic at the hospital where you are to be admitted, as a member of our staff will brief you on the procedure and/or treatment relating to your admission.
You will be advised on the financial considerations in respect of your hospitalisation, such as how to obtain pre-authorisation for your admission, the amount of cover afforded you by your medical scheme and any exclusion of benefits that may apply to you.
You will also be assisted with the completion of any necessary documentation to ensure a speedy and hassle-free admission to hospital.
This service is offered free of charge.
